The Health and Wellbeing Department deliver a range of programmes to improve the health of our participants. We target those most in need and utilise community-based, person-centred approaches to deliver excellent programmes. We work with a range of funders including Wandsworth Council, South West London CCG, and charities such as Macmillan and Mind. We have developed considerably in the past few years and are looking to continue that growth in future. 

 This post will lead our successful and growing Social Prescribing service. We deliver an enhanced model of social prescribing which includes specialist Link Workers, PCN Link Workers, and Health Coaches. The postholder will be responsible for leading, growing, and delivering all aspects of the service. This is a wide-reaching role that works with a number of stakeholders across the borough. 

 We are looking for someone who is an experienced leader with direct experience in managing health-related projects. You will need to work across a wide range of service areas including budgets, staffing, referral pathways, delivery models, and monitoring and evaluation. You should be comfortable communicating with senior managers, operational teams, and the public, and able to quickly build effective relationships with all our stakeholders.

Job Purpose

  • To lead and develop our Social Prescribing Service, including the PCN model, specialist Link Worker roles, Health Coaches, and Wandsworth Council’s Front Door Service pilot.

Role Specific:

  • Use excellent leadership skills to deliver high-quality service and seek opportunities for future growth.
  • Successfully deliver and develop an enhanced model of social prescribing, managing all aspects of this service.
  • Develop partnerships and relationships with key stakeholders including Clinical Directors, Wandsworth CCG, Wandsworth Council, and VCS Leaders
  • Ensure the safe and effective delivery of assessments for clients and patients referred to the service.
  • Manage the social prescribing grant fund and micro-commissioning pot, distributing funds in line with Enable LC guidelines and any contract specifications.
  • Manage a volunteering programme to support social prescribing clients. Measure the success and impact of this programme.
  • Ensure referral systems are in place and appropriate and work with providers to develop tools to meet our needs.

Business Development:

  • Take a strategic approach to service growth and improvement considering resources available and the needs of the organisation.
  • Identify and realise opportunities for developments within social prescribing, including expanding geographic reach, adding additional services, or broadening target groups.
  • Network with other social prescribing service managers, learning and sharing best practice to develop and improve our service.
  • Manage portfolio of contracts and work programmes ensuring they reach their targets, are appropriately staffed, complete all reports, financial monitoring and all third-party agreements are in place and compliant.
  • Be an advocate for social prescribing with partners and stakeholders. Support regional and national policy and strategy developments.
  • Prepare and contribute to the Health and Wellbeing Team’s work programme, supporting colleagues to deliver annual work plans and events, assisting with bids and tenders, as well as contributing to relevant strategies and action plans.

 Staffing:

  • Line management and development of staff within the social prescribing service.
  • Ensure staffing the structure is appropriate for the needs of the service and within budgets available.
  • Recruit and support Social Prescribing staff, professionals, and volunteers as appropriate to support the delivery of the Social Prescribing Services work programme, including supporting them in their professional development as appropriate.

 Finance & Budgets:

  • Manage the services financial performance. Monitor and document income and expenditure according to Enable LC policy and procedure.
  • Develop appropriate budgets for new developments with Head of Health and Wellbeing.
  • Ensure programme budgets are monitored in line with requirements of funders.

Monitoring & Evaluation:

  • Produce reports on social prescribing service, using both qualitative and quantitative statistical information as required. Consider further opportunities for sharing data and reports.
  • Support the development of systems to monitor performance ensuring the achievement of specified quality standards and performance measures.

 Marketing & Promotion:

  • Working with the Marketing and Communication Department prepare and deliver strategies and plans for programmes that encompass a variety of modes including; press releases, social media, the website and attending community events.
  • Seek to enhance and develop the reputation of Enable Health and Wellbeing. This should include speaking opportunities, citations in strategies and policies, award nominations.

  • Evening and weekend working as required.
In depth knowledge of personalised care, social prescribing, and health coaching including different models of delivery. 
Knowledge of relevant national, regional and local policies and initiatives.                                                                                                                                             
Knowledge of behaviour change models and understanding of the benefits of a person-centred approach to behaviour change. 
Experience  
Experience of working within a related environment, including experience within a social prescribing programme 
Experience of working in a leadership role, contributing to strategic developments within a programme, organisation, or project.  
Experience of leading on funding bids, or contract tenders, and working with funders to negotiate contract specifications.  
Experience of financial planning and effective budget management. 
Project management experience, preferably relating to working with partner organisations and planning, delivering and evaluating projects.
 
Experience of working within health and wellbeing sector and working with partner organisations
 
Experience of working with people with long term conditions, in particular mental health conditions.
 
Experience of recruiting, deploying, supervising and training staff at different grades and in a variety of roles. 
Skills & Ability 
Excellent communication and interpersonal skills, with the ability to converse with key people (including clients and the public) and develop new networks using a range of methods, including; oral, written, telephone and in meetings. 
Strong IT and web-based skills, with experience of using all applications within the Microsoft Office suite. 
Excellent evaluation skills with the ability to bring together multiple data sets and analyse data. Ability to present this data for a variety of audiences. 
Ability to work alone and effectively prioritise workload to meet the demands of the job in a timely manner. 
Able to work effectively with others in a team openly and collaboratively, valuing their differences creating a working environment which helps achieve goals 
Qualifications  
Holds an appropriate degree or equivalent experience in health (or similar) to demonstrate expertise. 
Personal qualities

Ambitious to grow and develop services, staff, and yourself. Driven to achieve targets and success.
