Social Prescribing for Young People in East Merton
If you are 13- 19 years old (or up to 25 if you have an EHCP) and live in East Merton and either need to manage your weight or your mental health, we can help you find the motivation to make positive changes to feel better.
Social Prescribers are here to listen to you and help you to take control of your own health, make lifestyles changes and decide what you would like to do to feel healthier – mentally and physically. We can help with managing stress, loneliness, meeting new people, learning new skills, exercising, or learning more about employment, benefits, housing and legal advice.
You can be connected to us by talking with lots of people – your GP, school nurse, someone from Talk Off The Record, or even your youth worker.
Would you like to feel better about your health, self-confidence and your wellbeing? Maybe you feel anxious, down, angry or low? Or would you like to set new goals and meet new friends?
How does it work?
We will:
- Explore what is important to you in your life and your wellbeing
- Identify local activities or services that you will find helpful and enjoyable
- Offer you support and encouragement to start using the services that can help you
You’ll be given six appointments which could be at your GP surgery, school or youth club , through Off The Record in Harris Academy Merton and St Marks Merton, Pollards Hill and Phipps Bridge.
How do I join?
You can be connected to us by talking with lots of people – your GP, school nurse, someone from Talk Off The Record, or even your youth worker. Your sessions can be face to face in East Merton GP surgeries or other easy-access spaces, or even over phone or phone call. At the end of the appointment you and your Link Worker will discuss ways to help that suit you and what you might need in future. We will keep in touch with you in case you’d like further support

If you are a GP and wish to refer to us please send an untimed appointment to Poppy with a reason for the referral on EMIS. Further info – poppy.tomlinson1@nhs.net
You must be aged 13 – 19 (or up to 25 if you have additional support needs) and live in Merton to access the service.
We can offer access translators upon request.
