Social Prescribing for Young People in East Merton & Morden

Would you like to feel better about your health, self-confidence and your wellbeing? Maybe you feel anxious, down, angry or low? Or would you like to set new goals and meet new friends? 

Social Prescribers are here to listen to you and help you decide what you would like to do to feel healthier – mentally and physically.

We can help with managing stress, loneliness, meeting new people, learning new skills, exercising, or learning more about employment, benefits, housing and legal advice.

We do this by:

  • Exploring what is important to you in your life and your wellbeing
  • Identifying local activities or services that you will find helpful and enjoyable
  • Offering you support and encouragement to achieve your goals

How do I join?

You can be connected to us by talking with lots of people – your GP, school nurse, someone from Talk Off The Record,  your youth worker.

What is the offer?

You can have six free sessions with a Social Prescribing link worker at an easy access venue where you feel comfortable. This could be at your GP surgery, school or youth club, Off The Record, or by phone.

We can offer access translators upon request.

Please note: You must be aged 13 – 19 or up to 25 if you have additional support needs and live, work or go to school or be registered with a GP in East Merton or Morden to access the service.
